NOTE: Background check MUST be completed prior to beginning of rotations, the background check may take several weeks; therefore you should begin the process as soon as possible. FAILURE TO COMPLETE A BACKGROUND CHECK PRIOR TO START of ROTATIONS WILL RESULT IN A DELAY IN BEGINNING ROTATIONS, WHICH MAY IMPACT YOUR ABILITY TO GRADUATE ON TIME

NOTE: ALL INFORMATION MUST BE COMPLETED ON THIS FORM, INCOMPLETE FORMS WILL RESULT IN DELAY IN BEGINNING ROTATIONS WHICH MAY IMPACT YOUR ABILITY TO GRADUATE ON TIME

Health screening includes:

negative two-step TB test prior to IPE2 (hospital rotation); followup negative TB test one year later prior to APE rotations

if positive TB test, must provide documentation of normal chest x-ray (and completion of TB therapy if deemed necessary)

documented immunity to: measles, mumps, rubella, and varicella (completed vaccination schedule and/or titers documenting immunity)

tetanus completed immunization with a booster within 10 years of date of rotations

medical clearance (i.e. physical exam signed by a physician or physicians assistant or nurse practitioner)
